Employee and Physician Assistance Program Coordinator, LMFT - Kaiser Permanente Santa Clara, Santa Clara, CA

Provide assessment and short-term, solution focused counseling and/or appropriate resources and referrals to other services. The assessment includes a care plan for follow-up. If appropriate, clients are referred to other programs as needed, including those within Kaiser Permanente (e.g., Chemical Dependency Services, Social Services, Mental Health, Health Education, and Smoking Cessation Counseling) as well as those in the community. 

Early Start Specialist, LMFT - Kaiser Permanente Santa Clara, Santa Clara, CA

Assessment of pregnant patients who have completed a self-administered questionnaire that screens for risk of alcohol, tobacco and illicit drug use during pregnancy as well as taking a medical and psychosocial history. Patients also provide a urine sample for toxicology testing with written consent. Gathering of detailed information about past and present alcohol, tobacco and drug use, and provide education and appropriate referrals to other services. The assessment concludes with a diagnosis and care plan for follow-up. If appropriate, patients are referred to other programs as needed, including those within Kaiser Permanente (e.g., Chemical Dependency Services, Social Services, Mental Health, Health Education, and Smoking Cessation Counseling) as well as those in the community (e.g., self-help programs, residential treatment). Additionally, this position requires occasional, immediate, triage, social work assessments from MD referrals for patients within the OB/GYN clinic.

 

Marriage & Family Therapist Intern - San Luis Obispo Child Development Center, San Luis Obispo, CA

Provided therapeutic counseling, play therapy and intervention to the Center’s children and parents who had experienced abuse including; physical assault, sexual abuse, emotional abuse, neglect and substance abuse. Many of the Center’s population were involved with Child Welfare, CPS and CASA. Performed psycho-social assessments, individualized treatment planning, assessment- team presentations for Medi-Cal approval and copious documentation. Maintained a caseload of up to 40 children and families. Program Coordinator- Family Support Services of West Hawaii, Kailua-Kona, HI. Developed and implemented a “Positive Youth” after-school program designed to serve up to 30 children. The program included integration of the Search Institute’s 40 Developmental Assets as well as physical fitness, expressive arts, weird science and numerous activities that focused on cultural and environmental issues.

 

Intern/Trainee - Family Services Agency of the Central Coast, Santa Cruz, CA

Counseled clients on parenting, sexual orientation, depression, job loss, alcohol/drug use and/or family issues due to past problems of abuse. Addressed the challenges of clients confronting issues, while often struggling to maintain jobs and relationships. Helped define and develop new coping strategies/behaviors. This internship included individual and group supervision.

 

Lead Inpatient Therapist - Watsonville Community Hospital Alcohol/Drug Treatment, Watsonville, CA

Provided treatment planning and implementation for all in-patient clients, supervised part-time and volunteer counseling staff, conducted individual counseling sessions for all in-patient clients, and facilitated group sessions incorporating a psycho-social educational approach to family groups.

 

First Offense Clerk, DUI Counselor/Instructor  - Janus of Santa Cruz, Santa Cruz, CA

First Offense Clerk– Tracked and monitored all first offender files, interfaced with State agencies, and maintained ongoing quality of service and compliance with State licensing while ensuring client confidentially.

 

DUI Counselor/Instructor– Followed an outlined curriculum, facilitated group discussions, and conducted client assessments and individual counseling sessions.

 

TCA Counselor II - Sunflower House, Santa Cruz, CA

Managed the women’s program in a Therapeutic Community treatment milieu. Duties included treatment planning and implementation for program residents, caseload file maintenance, family reunification, interfacing with numerous agencies and professional organizations while maintaining ongoing quality of care and compliance with licensing as well as State/Federal confidentiality.

 

Group Home Manager and Day-Activity Program Supervisor - Bayside Centers, Santa Cruz, CA

Group Home Manager– Managed a group home of 6 developmentally disabled adults with behavior problems and mental health issues so severe, the State of California deemed them to be the most management intensive. Chose each resident from Agnew’s State Hospital and wrote initial transition plan and on-going behavior plans for each client. Hired and trained and supervised all staff, conducted weekly staff meeting and treatment planning sessions. Maintained quality review of all client charts and interfaced with numerous agencies and professional organizations while maintaining ongoing quality of care and compliance with licensing as well as State/Federal confidentiality.

 

Day-Activity Program Supervisor– Provided Therapeutic Behavioral Services (TBS) for 30 developmentally disabled adults whose behavioral issues ranged from moderate to severe. The goal of the day program was to help clients avoid being placed in a higher level of care or to help the clients make a successful move to a lower level of care i.e. a less management intensive setting or even transitioning into independent living. This included developing and implementing behavioral plans, ensuring that interventions and strategies focused on improved self-management, self-awareness, and communication skills with a focus on positive reinforcement of desirable behaviors.
